Registration Fees & Form

The following  registration form must be completed and emailed. You are also required to print and complete the roster form and permission form (located on team forms page) and send them along with payment in the amount of $850.00.   Roster forms should be sent to us no later than October 21, 2011.

Cheques are to be made out to: Wellesley St. Clements Silver Stick.
